filesWhen you register at the surgery you will be asked to complete a GMS1 form. You will also be given a health questionnaire, which must be completed as fully as possible and returned with your GMS1 form. You can also register with the practice on-line by following the links below.

The practice boundary covers Hale Barns and parts of Hale and Timperley. Please ask our administration team for further details.

NEW REGISTRATIONS

If you are interested in registering, please fill out the following registration forms IN FULL, and bring these with you to the practice along with the following requested information.

Please ensure that you fill in ALL the details requested as failure to do so may delay your registration.

If you are from the UK:

  1. GMS1 Purple form
  2. Health Questionnaire

ALL FORMS MUST BE COMPLETED FOR REGISTRATION

NOTE: If you are a parent registering your child, or are under 18 years of age please
bring in any immunization history you may have.

If you are NOT from the UK

  1. GMS1 Purple form
  2. Health Questionnaire
  3. Proof of NHS entitlement (Work permit/visa etc.)
